Sharon, a New York state “snowbird” walked into the
office with a sense of urgency. She did not have urgency
of urination; however she desired the urine leakage to
stop every time she tried to swing her golf club. “Do
you have any idea how embarrassing it is to have the
urine showing through your pants” she asked me.” I need
something fast, I do not want any surgery and I do not

There are several injections for incontinence which
can be given in the office. Sometimes bladder infections
can cause incontinence and could be treated with an
injection of an antibiotic. Here we will be discussing
injections in the bladder or urethra (tube urine goes
through to the outside).
Botox in the bladder may work if you can’t hold the
urine till the bathroom at the ninth hole, however is
usually not indicated when you leak with activity.
Sharon agreed to be examined and have some testing
done. She was in good shape and her vagina and urethra
were well supported. When Sharon underwent urodynamic
testing, a test where the bladder is filled and being
monitored and challenged, even the smallest cough or
exertion caused leakage of urine.
Also on cystourethroscopy, where the inside of the
bladder and urethra can be viewed in the office, the
opening to the urethra from the bladder stayed wide
Since the exit from the bladder entering the urethra
stayed wide open, urine was allowed to escape when
Sharon would get up from a chair, walk or try to hit her
famous golf swing.
Even though a surgery, inserting a small piece of
polypropylene, a sling, would have been an option for
Sharon, she was adamant about not desiring surgery and
getting it fixed quickly before another embarrassing
moment at the club.

Peri-urethral (meaning around the tube you urinate
from) injection therapy can bulk or decrease the
diameter of the upper urethra. In other words the tube
connecting the bladder to the outside world now has
shutters which do not completely close. When the
urethral opening from the bladder stays wide open and is
without any urine holding power, peri-urethral bulking
can make a big difference. If the urethra is also very
mobile, a sling would work better.
The most common complications from peri-urethral
injection or bulking therapy are UTI (Urinary Tract
Infection) and urinary retention.
There are several materials that can be used.
Macroplastique® is a cross-linked polydimethylsiloxilane
and has been used for years without major complications
and with a good success rate. Macroplastique® is
hypoallergenic. At a 6-8 month follow up 37% of patients
reported a cure and 65% an improvement.
Sharon was cured with her office injection and can
now comfortably swing her golf club without having to
worry about leaking through her pants.
